ტორაგამა® 20 მგ ტაბლეტები არის შარდმდენი და სისხლის წნევის დამწევი საშუალება და მიეკუთვნება მარყუჟოვანი ტიპის დიურეზულ საშუალებებს. 

ტორაგამა® 20 მგ ტაბლეტები გამოიყენება: 

სითხის   დაგროვების   სამკურნალოდ   ან   მისი   განმეორებითი   გამოვლენის პრევენციისათვის ქსოვილში (კარდიალური შეშუპება) და/ან სხეულში (ჩაქცევები), რაც გამოწვეულია გულის ფუნქციის დარღვევის შედეგად (გულის კუნთის სისუსტე).

 

2. რას უნდა მიაქციოთ ყურადღება ტორაგამა® 20 მგ ტაბლეტების მიღებამდე? ტორაგამა® 20 მგ ტაბლეტების მიღება არ შეიძლება:

- თუ პაციენტს აღენიშნება ჰიპერმგრძნობელობა აქტიური ნივთიერების, თორასემიდის მიმართ, მისი მონათესავე ნივთიერებების  (სულფონილშარდოვანა) ან ტორაგამა® 20 მგ ტაბლეტების შემადგენელი ნებისმიერი დამხმარე ნივთიერების მიმართ.

- თირკმლის უკმარისობისას შარდის პროდუქციის შეზღუდვით (ანურია).

- ღვიძლის ფუნქციის რთული დარღვევისას გონების დაკარგვის თანხლებით (ჰეპატიური კომა ან პრეკომა)

- სისხლის წნევის ძლიერი დაცემისას (ჰიპოტონია)

- სისხლის მოცულობის დეფიციტისას (ჰიპოვოლემია)

- ნატრიუმის ან კალიუმის დეფიციტისას (ჰიპონატრიემია, ჰიპოკალიემია)

- ძლიერი დარღვევები  შარდვის დროს (მაგ., პროსტატის ძლიერი გადიდებისას).

- ლაქტაციის პერიოდში.

 

საჭიროა განსაკუთრებული სიფრთხილე ტორაგამა® 20მგ ტაბლეტების მიღების დროს: ვინაიდან  ამ  დრომდე  არ მოიპოვება გამოყენების საკმარისი გამოცდილება, ამიტომ ტორაგამა® 20 მგ ტაბლეტები არ უნდა მიიღოთ შემდეგ შემთხვევებში:

 

- პოდაგრა.

- გულის აღგზნებადობისა და გამტარობის ძლიერი დარღვევები (მაგ., II ან III ხარისხის

SA-ბლოკი, AV-ბლოკი).

- მჟავა/ტუტოვანი ბალანსის ცვლილებები.

- ლითიუმით ან ზოგიერთი ანტიბიოტიკებით (ამინოგლიკოზიდები, ცეფალოსპორინი)

ერთდროული მკურნალობისას.

- სისხლის  სურათის  ძლიერი  ცვლილებებისას  (მაგ.,  თრომბოციტოპენია  ან  ანემია

პაციენტებში თირკმლის ფუნქციის დარღვევის გარეშე).

- თირკმლის ფუნქციის დარღვევა თირკმლის დამაზიანებელი ნივთიერებების გამო.

 

ბავშვები და მოზარდები

ბავშვებში და მოზარდებში არ არსებობს ტორაგამა®  20 მგ ტაბლეტების გამოყენების

გამოცდილება. ამიტომ ტორაგამა®  20 მგ ტაბლეტები არ გამოიყენება ბავშვებში და მოზარდებში.

 

ტორაგამა® 20 მგ ტაბლეტების გამოყენება სხვა მედიკამენტებთან ერთად

გთხოვთ აცნობოთ ექიმს ან ფარმაცევტს, თუ იღებთ ან ცოტა ხნის წინ მიიღეთ სხვა

მედიკამენტი, მათ შორის ურეცეპტოდ გაცემულიც.

 

გთხოვთ, მიაქციოთ ყურადღება, რომ ეს გაფრთხილება ვრცელდება ასევე მედიკამენტებზე, რომლებიც მიიღეთ ცოტა ხნის წინ.

 

ტორაგამა® 20 მგ ტაბლეტები აძლიერებენ სისხლის წნევის დამწევი სხვა მედიკამენტების მოქმედებას, განსაკუთრებით აგფ- ინჰიბიტორების მოქმედებას. თუ აგფ-ინჰიბიტორები გამოიყენება დამატებითი თერაპიის სახით ტორაგამა® 20 მგ ტაბლეტების მკურნალობისას ან მკურნალობის შემდეგ დაუყონებლივ შესაძლოა გავითარდეს სისხლის წნევის ძლიერი დაცემა.

ტორაგამა®   20  მგ  ტაბლეტებით  გამოწვეულმა  კალიუმის  დეფიციტმა  შესაძლოა გამოიწვიოს გვერდითი ეფექტების გაძლიერება და გახშირება ერთდროულად საგულე გლიკოზიდების გამოყენების დროს.

ტორაგამა®  20 მგ ტაბლეტებმა შესაძლოა შეამციროს დიაბეტის სამკურნალო მედიკამენტების მოქმედება.

პრობენიციდმა (პოდაგრას სამკურნალო მედიკამენტი) და ზოგიერთმა ანთების საწინააღმდეგო   მედიკამენტებმა   (მაგ.ინდომეტაცინი, აცეტილსალიცილის   მჟავა) შესაძლოა დაასუსტონ ტორაგამა®  20 მგ ტაბლეტების შარდმდენი და წნევის დამწევი მოქმედება.

მაღალდოზირებადი სალიცილატით  მკურნალობისას (ტკივილგამაყუჩებელი და რევმატიზმის სამკურნალო საშუალება) შესაძლოა აღინიშნოს მათი ცენტრალურ ნერვულ სისტემაზე ზემოქმედების გაძლიერება   ტორაგამა® 20მგ ტაბლეტების ზეგავლენის შედეგად.

ტორაგამა®  20 მგ ტაბლეტებმა, განსაკუთრებით მაღალი დოზირების გამოყენებისას, შესაძლოა გამოიწვიოს გვერდითი ეფექტების გაძლიერება:

ამინოგლიკოზიდურების ჯგუფის ანტიბიოტიკების (მაგ. კანამიცინი, გენტამიცინი, ტაბრამიცინი) სმენისა და თირკმელების დამაზიანებელი ეფექტი, ცისპლატინის პრეპარატების   (კიბოს   საწინააღმდეგო   საშუალებები),   ასევე   ცეფალოსპორინების (ინფექციის სამკურნალო საშუალება) თირკმელების დამაზიანებელი ეფექტი.

თორაგამა® 20  მგ  ტაბლეტებმა  შესაძლოა  გააძლიერონ  თეოფილინის  (ასთმის სამკურნალო   საშუალება)   მოქმედება,   ასევე   გააძლიეროს   კურარეს   მსგავსი

მედიკამენტების კუნთების მომადუნებელი ეფექტი.

 

შარდმდენმა  მედიკამენტებმა,  ასევე  თირკმელზედა ქერქის  ჰორმონებმა (მინერალო და გლუკოკორტიკოიდები, მაგ. კორტიზონი) შესაძლოა გამოიწვიონ ტორაგამა® 20 მგ ტაბლეტებით გამოწვეული კალიუმის დაკარგვა.

ტორაგამა®  20 მგ ტაბლეტების და ლითიუმის ერთდროულმა გამოყენებამ შესაძლოა გამოიწვიოს  სისხლში  ლითიუმის კონცენტრაციის  მომატება  და  ასევე  გააძლიეროს

ლითიუმის მიერ გულისა და თირკმელების დამაზიანებელი ეფექტი.

ტორაგამა® 20 მგ ტაბლეტებმა შესაძლოა შეამცირონ არტერიების მგრძნობელობა სისხლძარღვების   შემავიწროვებელი   მედიკამენტების   მიმართ   (მაგ. ადრენალინი,

ნორადრენალინი).

ერთდროულად ქოლესტირამინით მკურნალობამ (სისხლში ცხიმის შესამცირებელი მედიკამენტი) შესაძლოა შეამციროს ტორაგამა® 20 მგ ტაბლეტების აბსორბცია კუჭ- ნაწლავის ტრაქტიდან და შედეგად შემცირდეს მისი მოქმედება.

 

ორსულობა და ლაქტაციის პერიოდი

ორსულობა

ამ  დრომდე  არ არსებობს საკმარისი კლინიკური გამოცდილება ტორაგამა®  20 მგ

ტაბლეტების ნაყოფზე მოქმედების შესახებ.

ტორაგამა®   20  მგ  ტაბლეტების  აქტიური  ნივთიერების  თორასემიდის  გამოყენებით ცხოველებზე ჩატარებულმა ექსპერიმენტებმა არ გამოავლინეს ნაყოფის დამაზიანებელი ეფექტი. აქტიური ნივთიერების მაღალი დოზირებით გამოყენებისას აღინიშნა ნაყოფზე და ასევე დედაზე დამაზიანებელი ეფექტი.

ვინაიდან  არ  მოიპოვება  სხვა  მონაცემები,  ამიტომ  ტორაგამა®  20  მგ  ტაბლეტების გამოყენება ორსულობის დროს შესაძლებელია მხოლოდ იმ შემთხვევაში, თუ ექიმი მკურნალობას მიიჩევს აუცილებლად საჭიროდ. ამავდროულად უნდა გამოყენებული იქნეს მხოლოდ ყველაზე დაბალი დოზირება.

გთხოვთ აცნობოთ ექიმს თუ გეგმავთ ორსულობას ან ხართ ორსულად.

 

ლაქტაცია

არ მოიპოვება მონაცემები იმის შესახებ გადადის თუ არა ტორაგამა® 20 მგ  ტაბლეტების აქტიური ნივთიერება თორასემიდი დედის რძეში ადამიანებსა თუ ცხოველებში. ამიტომ

ტორაგამა® 20 მგ ტაბლეტების გამოყენება ლაქტაციის პერიოდში არ შეიძლება. თუ ექიმი მიიღებს გადაწყვეტილებას მკურნალობის შესახებ, მაშინ უნდა შეწყდეს ლაქტაცია.

 

 

ზეგავლენა ავტომობილისა და მექანიზმების მართვის უნარზე

ტორაგამა®  20 მგ ტაბლეტების გამოყენებამ შესაძლოა გამოიწვიოს რეაქციის

ცვლილებები,  რის   შედეგადაც   მოსალოდნელია   ზეგავლენა   მოახდინოს   მაგ. სატრანსპორტო მოძრაობის დროს მართვის უნარზე და მექანიზმებთან მუშაობის უნარზე. ეს ზეგავლენა განსაკუთრებით შესაძლოა გამოვლინდეს მკურნალობის დასაწყისში, დოზირების გაზრდისა და პრეპარატის შეცვლის დროს ან დამატებითი მედიკაციის გამოყენებისას, ასევე ალკოჰოლის გამოყენებისას.

 

3. ტორაგამა® 20 მგ ტაბლეტების გამოყენების წესი.

მიიღეთ თორაგამა®  20 მგ ტაბლეტები ყოველთვის ექიმის დანიშნულების მიხედვით. გთხოვთ მიმართოთ ექიმს ან ფარმაცევტს თუ რაიმეში არ ხართ დარწმუნებული.

თუ ექიმი არ დაგინიშნავთ სხვაგვარად, რეკომენდებული დოზირება შეადგენს:

მითითება:

წინამდებარე დოზირება აქტიური ნივთიერების მაღალი შემადგენლობის გამო არ არის

რეკომენდებული მკურნალობის დაწყებისათვის.

მკურნალობა იწყება დღეში 5 მგ ტორასემიდით. ეს დოზირება არის შემანარჩუნებელი დოზირება.

დღეში 10 მგ ტორასემიდით მკურნალობა ნაჩვენებია, როდესაც ჩვეულებრივ დღეში 5 მგ ტორასემიდი არასაკმარისად ეფექტურია.

ასეთ შემთხვევებში გამოიყენება დღეში 10 მგ ტორასემიდი, არასაკმარისი ეფექტის

შემთხვევაში შესაძლებელია დაავადების სურათის სირთულის შესაბამისად დოზირება გაიზარდოს ტორაგამა®  20 მგ ტაბლეტების 1 ტაბლეტამდე დღეში (შეესაბამება 20 მგ ტორასემიდს). 

დაბალი დოზირებისათვის განკუთვნილია ტაბლეტები აქტიური ნივთიერების დაბალი შემცველობით. 

პაციენტებში ღვიძლის შეზღუდული ფუნქციით

ღვიძლის შეზღუდული ფუნქციის მქონე პაციენტებში მკურნალობა უნდა ჩატარდეს სიფრთხილით, რადგან შესაძლოა აღინიშნოს სისხლში თორასემიდის კონცენტრაციის გაზრდა.

 

ხანდაზმული პაციენტები

ხანდაზმულ პაციენტებში არ არის საჭირო დოზირების ცვლილება.

 

ბავშვები და მოზარდები

არ მოიპოვება ტორაგამა®  20 მგ ტაბლეტების ბავშვებში და მოზარდებში გამოყენების

გამოცდილება. ამიტომ ტორაგამა®  20 მგ ტაბლეტები არ გამოიყენება ბავშვებში და მოზარდებში.

 

მიღების ხანგრძლივობა

ტორაგამა® 20 მგ ტაბლეტები როგორც წესი მიიღება მთელი სიცოცხლის მანძილზე ან ქსოვილში სითხის დაგროვების (ედემა) აღმოფხვრამდე. მკურნალობის ხანგრძლივობა

განისაზღვრება ექიმის მიერ.

 

გთხოვთ მიმართოთ ექიმს ან ფარმაცევტს, თუ გაქვთ შეგრძნება, რომ ტორაგამა® 20 მგ ტაბლეტები თქვენზე მოქმედებს ძლიერად ან პირიქით სუსტად.

 

თუ მიიღეთ ტორაგამა® 20 მგ ტაბლეტების ჭარბი რაოდენობა

ტორაგამა®  20 მგ   ტაბლეტების შემთხვევით ან განხრახ ჭარბი დოზირების მიღებამ

შესაძლოა  გამოიწვიოს  ორგანოზმიდან  მარილისა  და  სითხის  ჭარბი  რაოდენობით

გამოდევნა, შესაბამისად გამოიწვიოს გონების დაკარგვა, დაბნეულობა, სისხლის წნევის დაცემა, სისხლის მიმოქცევის კოლაფსი და კუჭ-ნაწლავის გართულებები.

ასეთ შემთხვევაში დაუყონებლივ უნდა მიმართოთ ექიმს, რომელიც მიიღებს საჭირო ზომებს. 

 

თუ დაგავიწყდათ ტორაგამა® 20 მგ ტაბლეტების დოზირების მიღება

თუ მიიღეთ დაბალი დოზირება ან დაგავიწყდათ რეკომენდებული დოზირების მიღება შესაძლოა გამოვლინდეს ტორაგამა®  20 მგ ტაბლეტების ეფექტის შემცირება. ამან კი შესაძლოა  გამოიწვიოს  ჩივილების  სურათის  გაუარესება,  მაგ. წონაში  მატება  ან ქსოვილში სითხლის ძლიერი დაგროვება.

 

პრეპარატი  უნდა  მიიღოთ  გახსენებისთანავე.  თუ  ახლოვდება  მომდევნო  დოზირების მიღების დრო, მაშინ არ უნდა მიიღოთ გამოტოვებული დოზა. ასეთ შემთხვევაში უნდა განაგრძოთ მკურნალობა რეკომენდებული დოზირების რეჟიმის მიხედვით.

 

თუ გსურთ ტორაგამა® 20 მგ ტაბლეტების მიღების შეწყვეტა

მკურნალობის შეწყვეტის ან ნაადრევად დასრულების შემთხვევაში შესაძლოა აღინიშნოს

დაავადების  სურათის  გაუარესება.  არავითარ  შემთხვევაში  არ  უნდა  შეწყვიტოთ ტორაგამა® 20 მგ ტაბლეტების მიღება ექიმთან კონსულტაციის გარეშე ან ნაადრევად.

დამატებითი  კითხვების  შემთხვევაში  აღნიშნული  მედიკამენტის  გამოყენების შესახებ მიმართეთ ექიმს ან ფარმაცევტს.

 

4. მოსალოდნელი გვერდითი ეფექტები

ნებისმიერი  მედიკამენტის  მსგავსად  ტორაგამა®   20  მგ  ტაბლეტებმაც  შესაძლოა გამოიწვიონ გვერდითი ეფექტები, თუმცა არა ნებისმიერ პაციენტში. ტორაგამა®  20 მგ ტაბლეტების  ან  სხვა  მარყუჟული  დიურეზულების  დამახასიათებელი  არასასურველი

ეფექტები აღწერილია შემდგომში.

გვერდითი ეფექტები შეფასებულია მათი გამოვლენის სიხშირის მიხედვით: 

ძალიან ხშირი:

ვლინდება 10 პაციენტიდან 1-ზე მეტში

ხშირი:

ვლინდება 10 პაციენტიდან 1- ზე  ნაკლებში, მაგრამ 100

პაციენტიდან 1- ზე მეტში.

არახშირი:

ვლინდება 100 პაციენტიდან 1-ზე ნაკლებში,  მაგრამ  1000

პაციენტიდან 1 -ზე მეტში

იშვიათი:

ვლინდება 1000 პაციენტიდან 1-ზე ნაკლებში, მაგრამ 10.000

პაციენტიდან 1- ზე მეტში

ძალიან იშვიათი:

ვლინდება  10.000  პაციენტიდან  1 -ზე  ნაკლებში,  ერთეული

შემთხვევების ჩათვლით.


მნიშვნელოვანი  გვერდითი  ეფექტები  ან  სიმპტომები,  რომლებსაც  უნდა  მიაქციოთ ყურადღება:

თუ  შემდეგში  აღწერილი  გვერდითი  ეფექტებიდან  გამოვლინდება  რომელინა,  უნდა შეწყვიტოთ ტორაგამა® 20 მგ ტაბლეტების მიღება და მიმართოთ ექიმს დროულად.

 

ნივთიერებათა ცვლის და კვების დარღვევები

ხშირი:მჟავა-ტუტოვანი  ბალანსის  დარღვევის  გაძლიერება  (მეტაბოლური  ალკალოზი).

კუნთების სპაზმი (განსაკუთრებით მკურნალობის დასაწყისში). სისხლში შარდოვანას და გლუკოზის,  ასევე  ცხიმმჟავას (ტრიგლიცერიდები,  ქოლესტერინი)  კონცენტრაციის

მომატება. სისხლში კალიუმის რაოდენობის დეფიციტი (ჰიპოკალიემია) ერთდროულად კალიუმის გარეშე კვების დროს, ღებინების, ფაღარათის დროს, ჭარბი რაოდენობით შარდმდენი საშუალების გამოყენებისას, ასევე ღვიძლის ფუნქციის დარღვევის მქონე პაციენტებში. გამოყენებული დოზირების ოდენობის და მკურნალობის ხანგრძლივობის შესაბამისად შესაძლოა გამოვლინდეს მარილისა და სითხის ბალანსის დარღვევები, განსაკუთრებით,  მაგ. სისხლის  მოცულობის  შემცირება  (ჰიპოვოლემია),  სისხლში კალიუმის და/ან ნატრიუმის დეფიციტი (ჰიპოკალიემია და/ან ჰიპონატრიემია).

 

გულის დარღვევები

ძალიან იშვიათად: სისხლის   გასქელების   შედეგად   შესაძლოა   გამოვლინდეს

თრომბოემბოლიური გართულებები, დაბნეულობა, სისხლის წნევის დაცემა (ჰიპოტონია),

ასევე  სისხლის  გამტარობის  დარღვევები  და გულის  კუნთში  და ტვინში სისხლის გამტარობის შემცირება. ამ მდგომარეობამ შესაძლოა გამოიწვიოს მაგ. გულის რითმის დარღვევა (არითმია), მკერდის  არეში სივიწროვის  შეგრძნება (სტენოკარდია),  მწვავე გულის ინფარქტი ან უეცრად გონების დაკარგვა (სინკოპე).

 

კუჭ-ნაწლავის ტრაქტის დარღვევები

ხშირი:  კუჭ-ნაწლავის გართულებები (მაგ. მადის დაკარგვა (უმადობა), კუჭის ტკივილები,ღებინება, გულისრევა,  ფაღარათი  (დიარეა),  ყაბზობა  (ობსტიპაცია),  განსაკუთრებით

მკურნალობის დასაწყისში.

ძალიან იშვიათად: კუჭქვეშა ჯირკვლის ანთება (პანკრეატიტი).

 

თირკმელებისა და საშარდე გზების დარღვევები

არახშირი: სისხლში შარდოვანას და კრეატინინის მომატება. შარდვის დაბრკოლებისას

შესაძლოა აღინიშნოს შარდის შეკავება.

ღვიძლისა და ნაღველის დარღვევები

ხშირი: ღვიძლის ზოგიერთი ფერმენტების მომატება (გამა-Gთ).

 

კანისა და კანქვეშა ქსოვილის დაავადებები

ძალიან იშვიათად: ალერგიული რეაქციები, როგორიცაა, მაგ. ქავილი (პრურიტუსი), კანის ანთებითი    ცვლილებები (ეგზანთემა), სინათლის    მიმართ    მგრძნობელობა

(ფოტომგრძნობელობა), კანის რთული რეაქციები.

 

სისხლისა და ლიმფური კვანძების სისტემის დაავადებები

ძალიან  იშვიათად:  სისხლის  წითელი  და  თეთრი  უჯრედების  და  თრომბოციტების რაოდენობის შემცირება (თრომბოციტოპენია, ერითროციტოპენია და ლეიკოპენია).

 

ზოგადი დარღვევები და გართულებები გამოყენების ადგილას:

ხშირი:  თავის  ტკივილი,  თავბრუსხვევა,  დაღლილობა,  სისუსტე  (განსაკუთრებით მკურნალობის დასაწყისში).

არახშირი: პირის სიმშრალე, დაბუჟების და სიცივის შეგრძება კიდურებში (პარესთეზია). ძალიან  იშვიათად:  მხედველობის დარღვევები,  ყურებში  შუილი  (ტინიტუსი),  სმენის დაკარგვა.

 

გვერდითი ეფექტების მკურნალობა

თუ შენიშნავთ ზემოთ აღნიშნულ გვერდით მოვლენებს დროულად უნდა აცნობოთ ექიმს, რომელიც შეაფასებს მოვლენათა სირთულეს და მიიღებს საჭირო სამკურნალო ზომებს.

თუ უეცრად ვლინდება რომელიმე გვერდითი ეფექტი და გართულდება დაუყონებლივ უნდა მიმართოთ ექიმს, რადგან ზოგიერთი მოვლენა შესაძლოა საშიში იყოს სიცოცხლისათვის. ექიმი მიიღებს გადაწყვეტილებას სამკურნალო ზომების და ასევე მკურნალობის გაგრძელების შესახებ.

ჰიპერმგრძნობელობის  პირველივე  ნიშნებისას  უნდა  შეწყვიტოთ  ტორაგამა®      20  მგ ტაბლეტების მიღება და აღარასდროს აღარ უნდა მიიღოთ ეს პრეპარატი შემდგომში.

გთხოვთ აცნობოთ ექიმს ან ფარმაცევტს, თუ შენიშნავთ ისეთ გვერდით მოვლენას, რომელიც არ არის აღწერილი გამოყენების ინსტრუქციაში.

მედიკამენტი  ტორაგამა®   20  მგ  ტაბლეტების  გამოყენებამ  შესაძლოა  გამოიწვიოს დადგებითი შედეგი დოპინგის კონტროლის ტესტებზე.

 

5. ტორაგამა® 20 მგ ტაბლეტების შენახვის პირობები მედიკამენტი შეინახეთ ბავშვებისაგან მიუწვდომელ ადგილას.
