მუნი (ბღერი (scabies)) – ადამიანის კონტაგიოზური პარაზიტული დაავადება, რომელიც ხასიათდება ძლიერი ინტენსიური ქავილით; დაავადების გამომწვევია ბღერის ტკიპა Sarcoptes scabiei var. hominis.

დასნებოვნება ხდება დაავადებულ ადამიანთან პირდაპირი კონტაქტით, მუნითეთრეულით, ტანსაცმლით და პირადი მოხმარების საგნებით. დაავადების ყველაზე ხშირი წყაროა ოჯახის წევრები, მეზობლები და კონტაქტში მყოფი სხვა პირები; ასევე გადაეცემა სქესობრივი კავშირით. ინკუბაციური პერიოდი დაახლოებით 1–2 კვირა გრძელდება . კლინიკური გამოვლინებები განპირობებულია განაყოფიერებული მდედრობითი ტკიპის ჩანერგვით ეპიდერმისის რქოვან შრეში, სადაც იგი განათავსებს კვერცხებს. დაახლოებით 2 კვირაში, გაივლის რა განვითარების რამდენიმე სტადიას, კვერცხებიდან წარმოიქმნება პარაზიტის მწიფე ფორმები, რომლებიც ვრცელდებიან კანის ახალ უბნებში.

კლინიკური სურათი

მუნის ძირითადი სიმპტომია ღამის ქავილი, განპირობებული ტკიპების აქტიურობით ამ პერიოდში. გარდა განაკაწრებისა, აღინიშნება წვრილი პაპულოვეზიკულები, წვრილი ხაზოვნად დალაგებული ბუშტუკები და კვანძები. დაავადებისათვის პათოგნომურია “მუნის გასავლები” – მცირე ზომის სწორი ან დაკლაკნილი ხაზები, ბოლოში ბუშტუკით, სადაც იმყოფება მდედრობითი ტკიპა. ხვრელები ერთმანეთისგან 0,5-1 სანტიმეტრით არიან დაშორებულები. გამონაყარის დამახასიათებელი ლოკალიზაციაა კანის თხელი ნაზი უბნები: თითებშუა არეები, სხივ–მაჯის მომხრელი არე, მუცელი, სასქესო ასო, ბარძაყების შიდა ზედაპირი, სარძევე ჯირკვლები. მუნის დროს არ ზიანდება სახის კანი და თავის თმიანი ნაწილი. მუნი ხშირად რთულდება პიოდერმიით (ფურუნკულები, ექტიმები, იმპეტიგო).

დიაგნოზი ეყრდნობა ეპიდემიოლოგიურ ანამნეზს, ტიპიურ სიმპტომატიკასა და ლაბორატორიულ გამოკვლევებს.

მკურნალობა

დაავადება განკურნებადია, მაგრამ მკურნალობის პროცესი საჭიროებს დიდ ძალისხმევას. მკურნალობის შერჩევა ხდება ექიმი-დერმატოლოგის მიერ. მუნის სამკურნალოდ გამოიყენება გოგირდის მალამო, პერმეტრინი, სპრეგალი, სპეციალური სქემების მიხედვით. მკურნალობის შემდეგ აუცილებელია ავადმყოფის ორჯერადი გასინჯვა 1 კვირიანი შუალედით. პროფილაქტიკის მიზნით, სამედიცინო შემოწმებას ექვემდებარება ოჯახის ყველა წევრი და ავადმყოფთან კონტაქტში მყოფი ყველა პირი, განსაკუთრებით საბავშვო დაწესებულებებში.

განკურნება მიიღწევა 3 პირობის დაცვით:

ექიმის მიერ დანიშნული მკურნალობის სქემის ზუსტი შესრულება;
ავადმყოფთან კონტაქტში მყოფი ყველა პირის პროფილაქტიკური დამუშავება;
ჰიგიენური პირობების დაცვა. პირადი მოხმარების საგნებისა და საცხოვრებელი ადგილის დეზინფექცია.

პრევენცია

მუნის პრევენციის საუკეთესო საშუალებას წარმოადგენს ჰიგიენური პირობების დაცვა და სხვა პიროვნების პირადი მოხმარების ნივთების გამოყენებისგან თავის შეკავება. მუნის აქტიური გამოვლენის მიზნით ასევე მნიშვნელოვანია რუტინული ინსპექციების ჩატარება მოსახლეობაში; დაავადების აღმოჩენის შემთხვევაში, აუცილებელია მუნის კერების აღმოფხვრა, ინფექციის წყაროს სრული მკურნალობა, საკონტაქტო პირზე დაკვირვება და სადეზინფექციო ზომების განხორციელება.
